Objective To observe the therapeutic effect of Zhilong Huoxue Tongyu Capsules [capsules of Shuizhi（Hirudo） and Dilong（Lumbricus） to quicken the blood]combined with TCM syndrome identification for acute ischemic stroke. Methods Totally 45 cases of acute ischemic stroke were randomized into the treatment group and control group

with 30 and 15 in each. Both groups were given free fried Chinese medicine granules according to syndrome identification one dose daily. The treatment group was added with Zhilong Huoxue Tongyu Capsules three times daily. The therapeutic effect was observed after 14-day treatment. The scores of Canadian Stroke Scale（ CSS）

National Institutes of Health Stroke Scale（ NIHSS）

Barthel Index（ BI） and Modified Rankin Scale（ MRS） were evaluated. Results The total effective rate of the treatment group and control group was 93. 33% and 73. 34% respectively

with a significant difference between groups（ P < 0. 05）. The scores of CSS

BI and MRS were significantly improved in the treatment group after treatment（ P < 0. 01）. The scores of CSS and MRS were significantly improved in the control group after treatment（ P < 0. 01）. Conclusion Zhilong Huoxue Tongyu Capsules combined with TCM syndrome identification is effective for acute ischemic stroke and it can obviously improve the neurological deficit.
