摘要

基于“一气周流”理论探讨腹泻型肠易激综合征的病机，认为脾虚湿困、中气失于斡旋是本病发生的根本，同时肝郁左升无力、肺滞右降不及、肾寒气化不行，加重气机失常是本病发生、发展的关键；基于“一气周流”理论指导腹泻型肠易激综合征的治疗，则强调把握脾、肝、肺、肾四脏气机的整体变化，以恢复中气斡旋为核心，并根据气机运转的动态变化分别侧重于健脾复运、调肝解郁、宣肃肺气、温肾助阳等治法，多脏同调，促使一气周流通畅，进而治愈本病。

Abstract

The basic model of HUANG Yuanyu's “one

circulation” is that the central earth is the pivot， and the four

xiang

（象） are the wheel，with continuous

movement. This paper discussed the pathogenesis of diarrhea predominant irritable bowel syndrome （IBS-D） based on the theory of “one

circulation”. It is considered that the root of the disease is spleen deficiency leading to damp encumbrance， and center

failing to mediate. At the same time， the powerless

ascending in the left due to liver constraint， untimely

descending in the right caused by lung

stagnation， and failure of

transformation induced by kidney cold， all aggravating the

movement disorder， is the key to the progression of the disease. Therefore， based on the “one

circulation” theory， the treatment should grasp the overall changes of the

movement of the spleen， liver， lung and kidney， take the recovery of the rise and fall of center

as the core， and adopt the treatment methods of fortifying the spleen and helping the movement， regulating the liver and resolving constraint， diffusing and clearing lung

， warming the kidney and assissting

yang

according to the dynamic changes of the

movement and circulation， so as to regulate multiple

zang

（脏） simultaneously， promote the smooth circulation of one

， and furthermore cure the disease.
