摘要

目的

观察桃红四物汤合自拟补气养阴汤治疗糖尿病足气阴两虚兼瘀证患者的临床疗效及安全性，并探讨其可能的作用机制。

方法

84例糖尿病足气阴两虚兼瘀证患者随机分为治疗组及对照组各42例。对照组给予西医常规治疗。治疗组在此基础上再给予桃红四物汤合自拟补气养阴汤口服，每日1剂。两组疗程均为4周。比较两组患者治疗前后中医证候评分（包括主症评分和次症评分），检测空腹血糖（FBG）、氧化应激反应指标［包括丙二醛（MDA）、超氧化物歧化酶（SOD）及谷胱甘肽过氧化物酶（GSH-Px）］及炎症因子［包括超敏C反应蛋白（hs-CRP）、白细胞介素6（IL-6）及白细胞介素1β（IL-1β）］，治疗期间观察不良反应发生情况，治疗后比较两组患者临床疗效。

结果

治疗组临床疗效总有效率（90.48%）明显高于对照组（73.81%，

=0.046）。治疗后两组患者中医证候主症评分、次症评分、FBG、MDA、hs-CRP、IL-6及IL-1β水平均较本组治疗前明显下降，SOD、GSH-Px水平升高（

＜0.05），且治疗后治疗组各指标改善明显优于对照组（

＜0.05）。治疗期间治疗组出现2例（4.76%，腹部不适和皮疹各1例）、对照组出现1例（2.38%，腹部不适）不良反应，对症治疗后症状均消失，两组比较差异均无统计学意义（

＞0.05）。两组患者治疗前后血常规、尿常规、肝功能、肾功能等实验室检查均未见明显异常。

结论

桃红四物汤合自拟补气养阴汤联合西医常规治疗可有效提高糖尿病足气阴两虚兼瘀证患者临床疗效，改善临床症状，较单独常规西医治疗为优，其机制可能与缓解氧化应激反应及炎症反应有关。

Abstract

Objective

To investigate the effect and safety of

Taohong Siwu Decoction

（桃红四物汤） and self-made

Buqi Yangyin Decoction

（自拟补气养阴汤） in the treatment of patients with diabetic foot and

yin

deficiency and stasis syndrome， and to explore the possible mechanism.

Methods

A total of 84 diabetic foot patients with

yin

deficiency and stasis syndrome were randomly divided into treatment and control group， each with 42 cases. The control group was given conventional western medicine， while the treatment group was additionally

given oral

Taohong Siwu Decoction

and self-made

Buqi Yangyin Decoction

， one dose a day， both groups for four weeks. The traditional Chinese medicine （TCM） syndrome scores including main symptoms score and secondary symptoms score， fasting blood glucose （FBG）， oxidation reaction indicators including malondialdehyde（MDA）， superoxide dismutase （SOD） and glutathione peroxidase （GSH-Px）， and inflammatory factors including hypersensitive C-reactive protein （hs-CRP）， interleukin 6 （IL-6） and interleukin 1β （IL-1β） were assessed before and after treatment. The adverse reactions were observed during treatment， and the clinical effect was analyzed.

Results

The total effective rate of the treatment group was 90.48%， which was higher than 73.81% of the control group （

= 0.046）. After treatment， the main symptoms score， secondary symptoms score， and the levels of FBG， MDA， hs-CRP， IL-6 and IL-1β in both groups significantly decreased， while the levels of SOD and GSH-PX increased （

＜0.05）， and more improvements were found in the treatment group than in the control group （

＜0.05）. During the treatment， there was two cases （4.76%） of adverse events including one abdominal discomfort and one case of rash in the treatment group， one case （2.38%） of abdominal discomfort in the control group， all of which disappeared after symptomatic treatment. There was no statistically significant difference between groups in adverse reactions （

＞0.05）. There were no obvious abnormalities in laboratory tests such as blood routine， urine routine， liver function and renal function in both groups.

Conclusion

Taohong Siwu Decoction

and self-made

Buqi Yangyin Decoction

combined with conventional western medicine can effectively improve the clinical outcomes and reduce clinical symptoms in patients with diabetic foot and

yin

deficiency and stasis synrome than western medicine alone， which may be related to the alleviation of oxidative stress and inflammation.
