摘要

原发性膜性肾病是一种自身免疫性肾小球病，已成为成人肾病综合征最主要的病理类型，可归属中医学“水肿病”范畴。在脏腑风湿论和态靶辨治理论体系基础上，提出原发性膜性肾病从病机衍变上可分为“虚、伏、变、损”四个阶段，临床表现包含“虚、寒、湿、瘀、积、饮”六态。治疗上以麻黄附子汤合肾着汤为病靶主方，针对“免疫、病理、指标”三个内含靶标，根据六态灵活化裁，谨守“温阳化积，解表散寒”的治疗总则，构建原发性膜性肾病的态靶辨治体系。

Abstract

Primary membranous nephropathy （PMN） is an autoimmune glomerular disease， which has become the main pathological condition of nephrotic syndrome in adults， and belongs to “edema” in traditional Chinese medicine. Based on the theories of

zang

（脏腑） organs wind-damp， and state differentiation and target treatment， it is proposed that PMN can be divided into four stages according to the progression of the pathological mechanism， that is deficiency， latency， change， and depletion， and the six states of the clinical manifestations include deficiency， cold， damp， stasis， accumulation， and retention.

Mahuang Fuzi Decoction and Shenzhuo Decoction

（麻黄附子汤合肾着汤） is the main prescription targeting the disease. Aiming at the three contained targets of immunity， pathology， and indicators， the prescription should be flexibly modified based on the six states， and the principle of warming yang and removing accumulation， releasing the exterior and dissipating cold should be adhered to， in order to construct a state-target differentiation and treatment system for PMN.
