摘要

总结运用角药治疗肝硬化的临证经验，阐释其配伍特点。临证以土鳖虫-煅龙骨-煅牡蛎角药活血化瘀、软坚散结，治疗肝硬化结节、脾大；大腹皮-白茅根-椒目角药疏利三焦、气血水同治，治疗肝硬化腹水；茵陈-白术-桂枝角药温中健脾、利湿退黄，治疗肝硬化黄疸之阴黄证；金钱草-郁金-鸡内金角药清热利胆、化坚排石，治疗肝硬化合并胆囊炎、胆结石；浙贝母-荔枝核-皂角刺角药化痰散结、理气消癖，治疗男性肝硬化患者乳房发育。同时，注重三因制宜的基本原则，并结合现代研究，将角药或独立成方，或入复方使用，以便提高中医药治疗肝硬化的临床疗效。

Abstract

This paper summarized the clinical experience of treating liver cirrhosis with

jiao

（角） medicine （a triple drug combination）， and explained the characteristics of combining the medicinals. As

jiao

medicine， the combination of Tubiechong （

Eupolyphaga seu Steleophaga

）， Duanlonggu （

Fossilia Ossis Mastodi Praeparatum

） and Duanmuli （

Concha Ostreae Praeparatum

） is used in clinical practice to activate blood and dissolve stasis， soften hardness and dissipate masses in the treatment of nodules in the liver cirrhosis and splenomegaly. Dafupi （

Pericarpium Arecae

） and Baimaogen （

Rhizoma Imperatae

） in combination with Jiaomu （

Semen Zanthoxyli

） are used to soothe and promote the function of

sanjiao

（三焦）， treat the qi， blood and water simultaneously in treating ascites in the liver cirrhosis. Yinchen （

Herba Artemisiae Scopariae

）， Baizhu （

Rhizoma Atractylodis Macrocephalae

） and Guizhi （

Ramulus Cinnamomi

） together can warm the center and fortify the spleen， drain dampness and reduce jaundice， so as to treat yin jaundice associated with cirrhosis. The combination of Jinqiancao （

Herba Lysimachiae

）， Yujin （

Radix Curcumae

） and Jineijin （

Endothelium Corneum Gigeriae Galli

） has the function of clearing heat and promoting gallbladder function， softening hard masses and expelling stones， and can be used to treat liver cirrhosis with cholecystitis and gallstones. Zhebeimu （

Bulbus Fritillariae Thunbergii

）， Lizhihe （

Semen Litchi

） and Zaojiao （

Fructus Gleditsiae

） together can dissolve phlegm and dissipate masses， rectify qi and dispel lump， and is often used to treat gynecomastia in liver cirrhosis. At the same time， it is emphasized to give treatment individualized to patient， season and locality， and use

jiao

medicine alone or in compound formula based on modern research， so that the clinical effect of traditional Chinese medicine in treatment of liver cirrhosis could be further improved.
