摘要

血浊理论在脑小血管病的辨证治疗方面具有重要指导意义。通过阐述血浊的基本概念、性质、致病特点及血浊与诸病理因素的关系，提出在血浊理论指导下的脑小血管病“浊血伤髓”的发生学认识。明确了浊血伤髓是脑小血管病的核心病机，指出脑小血管病的发生是以肾精不足、髓海亏虚为前提条件，血浊壅盛、损伤脑髓为关键原因。因此在治疗上主张从血浊入手，在清化血浊、填精益髓的核心治法下，选择化浊益髓方为基础，结合调肝和气、理血通络、调理脏腑等方法改善脑小血管病的病情，为其辨证治疗提供思路。

Abstract

The theory of blood-turbidity is critical to the syndrome differentiation and treatment of cerebral small vessel disease （CSVD）. By expounding the basic concept， nature， pathogenic characteristics of blood-turbidity and its relationship with various pathological factors， this paper put forward the understanding of the occurrence of ‘turbid blood damaging marrow’ of CSVD under the guidance of blood-turbidity theory. The core pathogenesis of CSVD is turbid blood damaging marrow. For the occurrence of CSVD， the premise is kidney essence insufficiency and marrow sea depletion， while the key reason is the accumulation of blood-turbidity and the injury of brain marrow. Therefore， in the treatment， it is advocated to start with blood turbidity. It is suggested to treat CSVD from blood-turbidity， and with the core treatment method of clearing and removing blood turbidity and supplementing essence and boosting marrow，

Huazhuo Yisui Formula

（化浊益髓方） is selected as the basis； moreover， the methods such as regulating liver and harmonizing qi， regulating blood and unblocking collaterals， and regulating

zang

organs （脏腑） can be integrated to improve the condition of CSVD， so as to provide ideas for the syndrome differentiation and treatment of CSVD.
