This paper summarizes clinical experience in treating cough due to external pathogens during pregnancy based on the theory of simultaneous treatment of lungs and spleen. It is believed that such cough primarily involves the lungs and spleen, closely related to dysfunction in both organs. Treatment should consider the unique physiological conditions and pathogenesis of pregnant women, advocating simultaneous regulation of both lungs and spleen, which addresses cough relief while concurrently calming the fetus. For lung regulation, a combination of acrid-cool medicinals is recommended to disperse wind, clear heat, and eliminate pathogens; for spleen regulation, focus on strengthening the spleen, calming the fetus, and harmonizing

movement. Medicinal formulations should be light, clear, agile, and mild in nature. A self-prescribed basic formula for treating external-pathogen related cough during pregnancy was frequently used, modified according to individual symptoms, which can provide treatment ideas in clinic.
