摘要

总结情志病“气火从化”病机演变规律，认为心胆动摇为情志病发病关键，情志过极为情志病的重要诱因，肝郁气滞为情志病的初始病机，并可具有从气、从火两种病机演变途径。从气之证的演变规律为：肝郁气滞证→肝郁脾虚证→心脾两虚证→肝肾阴虚证；从火之证的演变规律为：肝郁气滞证→肝郁化火证→肝火上炎证→心肝火旺证→肝肾阴虚证。从火、从气演变后均可归属于肝肾阴虚之证，继而表现为肝肾阴虚证→阴虚火旺证→心肾不交证的转归过程。据此提出情志病辨证应以虚实寒热为统领，以脏腑、气血津液为核心，确立首安心胆、随证治之、扶正祛邪、未变先防的治疗原则，为情志病临床诊疗提供思路。

Abstract

This paper summarizes the evolutionary pattern of the pathomechanism of "

transforming into fire" in emotional disorders. It proposes that vacillation of the heart and gallbladder is the key pathogenic factor， while excessive emotional stimulation serves as an important trigger. The initial pathomechanism is stagnation of liver-

， which may evolve along two pathways： from

or from fire. The progression of the

pathway is： stagnation of liver-

→ liver-

stagnation with spleen deficiency → dual deficiency of heart and spleen → liver and kidney

yin

deficiency. The progression of the fire pathway is： stagnation of liver-

→ liver-

transforming into fire → upward flaming of liver-fire → exuberance of heart and liver-fire → liver and kidney

yin

deficiency. Both pathways eventually converge into liver and kidney

yin

deficiency， which further evolves into

deficiency with hyperactive fire and then disharmony between heart and kidney. On this basis， it is suggested that syndrome differentiation of emotional disorders should be guided by the principles of deficiency-excess and cold-heat， with focus on

， blood， and body fluids. The therapeutic strategy should emphasize stabilizing the heart and gallbladder， treating according to syndr

and eliminating pathogenic factors， and preventing progression at early stages. This provides insights for the clinical diagnosis and treatment of emotional disorders.
