摘要

基于“邪伏肺络”理论从虚、痰、瘀探讨肺泡蛋白沉积症的中医病机与治疗。认为肺泡蛋白沉积症的主要病机为脏腑阳气不足，因虚致实，导致肺脏局部痰浊内停，瘀血内阻，二者胶着，蛰伏肺络而发病。脏腑阳气不足为发病之根，痰瘀盘踞络道是发病和疾病进展的关键病理因素。治疗以益气升阳、豁痰祛瘀通络为主，起始发病期治以培土生金、扶正祛邪，进展期治以益气升阳、化痰祛瘀；疾病后期治以温肾填精，辅以化痰祛瘀。临证当参病症之虚实缓急，辨病位之所在，灵活加减，俾使阳气充足，痰瘀得祛，肺络得通，促使疾病痊愈。

Abstract

Based on the theory of "latent pathogen in the lung collaterals"， this paper explores the traditional Chinese medicine （TCM） pathogenesis and treatment of pulmonary alveolar proteinosis （PAP） from the perspectives of deficiency， phlegm， and blood stasis. It is proposed that the main pathogenesis of PAP lies in insufficiency of the visceral

yang qi

， which leads from deficiency to excess， resulting in internal retention of phlegm turbidity and obstruction of blood stasis in the lung. These two pathological factors intertwine and remain latent within the lung collaterals， thereby inducing disease. The insufficiency of visceral

yang qi

is regarded as the root cause， while the accumulation of ph

legm and blood stasis in the collaterals constitutes the key pathological mechanism in disease onset and progression. The therapeutic principles emphasize on tonifying

and elevating

yang

， resolving phlegm， removing blood stasis， and dredging the collaterals. In the initial stage， treatment should focus on "reinforcing earth （spleen and stomach） for generating metal （lungs and large intestine）" and supporting the healthy

to eliminate pathogens. During the progressive stage， reinforcing

yang

， and resolving phlegm and blood stasis are prioritized. In the late stage， the approach should shift to warming the kidney and replenishing essence， supplemented by resolving phlegm and removing stasis. Clinically， treatment should be flexibly adjusted according to the degree of deficiency and excess， the urgency of symptoms， and the location of the disease， so as to restore

yang qi

， eliminate phlegm and stasis， unblock the lung collaterals， and promote recovery.
