摘要

认为直肠癌术后低位前切除综合征病位在魄门，以肛周气血运行不畅、肠道传导糟粕失序之症状为急，以五脏功能失调致魄门失司之病机为本，并强调五脏之职各有轻重，其中以脾肾为重，以心肝肺为辅。诊疗思路强调“缓魄门之急，调五脏之本”，以疏调肛周局部气血、调节肠道传导功能为先，斡旋脾胃升降之机、重振肾气司便之能为重，疏理肝气郁滞不舒、畅调肺气壅塞不通、安定心神失养不宁为辅。局部治疗以求直达病灶，整体治疗注重审证求因。

Abstract

The paper holds that low anterior resection syndrome （LARS） after rectal cancer surgery is primarily associated with dysfunction at the

（魄） gate， characterized by impaired

and blood circulation around the anus and disordered intestinal transport of waste. The fundamental pathogenesis involves dysregulation of the five

zang

organs， leading to loss of control at the

gate. Among the five

zang

organs， the spleen and kidney are considered primary contri-butors， while the heart， liver， and lung play secondary roles. T

he therapeutic strategy emphasizes on "alleviating the acute disorder at the

gate and regulating the root of the five

zang

organs". Local interventions aim to restore

and blood flow around the anus and normalize intestinal transport function. The main focus is on harmonizing the ascending and descending functions of the spleen and stomach and enhancing the kidney's role in defecation control. Supportive interventions target stagnant liver

， obstructed lung

， and unsettled heart-mind to achieve holistic balance. Local treatment directly address the lesion， whereas systemic treatment prioritizes pattern differentiation and root-cause regulation.
