摘要

基于临床实践与刘完素“亢害承制，火极似水”理论，认为变应性鼻炎之受寒痒嚏、清涕如水可因火热所致，临证当整体与局部相参，明辨变应性鼻炎之寒热分属。对于火热所致者，当详察脏腑火热虚实之不同，阳明积热上蒸者，治以清胃泄火、通腑泄热，方予辛夷清肺饮或翘荷汤合宣白承气汤加减；肝胆风火上扰者，治以清肝利胆、疏风散邪，方予丹栀小柴胡汤合过敏煎加减；脾虚湿郁热蒸者，治以升阳散火、健脾祛湿，方予升阳益胃汤加减；肾虚水亏火旺者，壮水之主、以治阳光，方予引火汤合封髓丹加减。

Abstract

Based on clinical practice and LIU Wansu's theory of "harmful hyperactivity and responding inhibition， and extreme fire resembling water"， this paper proposes that the the cold-induced itching， sneezing， and profuse clear watery nasal discharge seen in allergic rhinitis may， in certain cases， be caused by fire-heat. In clinical practice， both systemic and local manifestations should be considered to clearly differentiate cold and heat patterns in allergic rhinitis. For cases caused by fire-heat， careful differentiation of the location and nature of

zang

fire-heat， including deficiency and excess patterns， is essential. For exuberant heat accumulated in the

yangming

（阳明） channel rising upward， treatment should focus on clearing the stomach and draining fire， unblocking the bowels， and dispersing heat， using modified

Xinyi Qingfei Decoction

（辛夷清肺饮） or

Qiaohe Decoction

（翘荷汤） combined with modified

Xuanbai Chengqi Decoction

（宣白承气汤）. For hyperactivity of liver and gallbladder wind-fire disturbing upward， treatment should emphasize clearing the liver and gallbladder， dispelling wind， and eliminating pathogenic factors， using modified

Danzhi Xiaochaihu Decoction

（丹栀小柴胡汤） combined with modified

Guomin Decoction

（过敏煎）. For spleen deficiency with dampness stagnation transforming into heat， treatment should aim to raising

and dispersing fire， strengthening the spleen， and eliminating dampness， using modified

Shengyang Yiwei Decoction

（升阳益胃汤）. For kidney deficiency with water depletion and exuberant fire， treatment should focus on "reinforcing the master of water to control blazing
