摘要

总结王书臣教授治疗慢性阻塞性肺疾病（COPD）的临证经验。认为三焦失衡、虚痰瘀结为COPD核心病机，以调畅三焦、通补兼施为治疗总则。临证重视扶正固本，脾肾同调，常用半夏泻心汤加减健脾化痰，二仙汤加减以补肾纳气；三焦宜畅，治应因势利导，常用荆防败毒散或麻杏石甘汤加减以宣透上焦之邪，真武汤或苓桂术甘汤加减以温化中焦水饮，己椒苈黄合宣白承气汤加减以通利下焦之滞；痰瘀难散，正气难复，治应益气活血、阴阳并调，常用补中益气汤合生脉散加减。随证灵活配伍，以达扶正固本，补而不滞，通而不伤之功。

Abstract

This paper summarizes professor WANG Shuchen's clinical experience in the treatment of chronic obstructive pulmonary disease （COPD）. It is considered that imbalance of the

sanjiao

（三焦）， together with deficiency， phlegm， and blood stasis binding together， constitutes the core pathogenesis of COPD， and that smoothing and regulating the

sanjiao

while combining dredging and tonification should be the overall therapeutic principle. In clinical practice， emphasis is placed on reinforcing vital

and consolidating the root， with simultaneous regulation of the spleen and kidney. Modified

Banxia Xiexin Decoction

（半夏泻心汤） is commonly used to strengthen the spleen and resolve phlegm， while modified

Erxian Decoction

（二仙汤） is applied to

tonify the kidney and promote

reception. Free flow of the

sanjiao

is essential， and treatment should follow the momentum of the disease in which modified

Jingfang Baidu Powder

（荆防败毒散） or

Maxing Shigan Decoction

（麻杏石甘汤） is often used to diffuse and vent pathogenic factors from the upper

jiao

， modified

Zhenwu Decoction

（真武汤） or

Linggui Zhugan Decoction

（苓桂术甘汤） to warm and transform water retention in the middle

jiao

， and modified combinations of

Jijiao Lihuang Decoction

（己椒苈黄汤） with

Xuanbai Chengqi Decoction

（宣白承气汤） to unblock stagnation in the lower

jiao

. When phlegm and blood stasis are difficult to resolve and vital

is hard to restore， treatment should focus on tonifying

， activating blood circulation， and regulating

yin

and

yang

simultaneously， commonly using modified

Buzhong Yiqi Decoction

（补中益气汤） combined with

Shengmai Powder

（生脉散）. Prescriptions are flexibly combined according to syndrome differentiation to achieve the effects of reinforcing the root， tonifying without causing stagnation， and dredging without damaging vital
