摘要

目的

观察健脾解毒方治疗木村-竹本闭合型慢性萎缩性胃炎（CAG）脾虚湿热证伴肠上皮化生的临床疗效。

方法

纳入180例木村-竹本闭合型CAG脾虚湿热证患者，随机分为治疗组及对照组各90例。治疗组给予健脾解毒方治疗，对照组给予维生素C、叶酸片联合六君子汤治疗，两组疗程均为6个月。比较两组治疗前后可操作的与胃癌风险联系的胃炎评估（OLGA）分期和可操作的与胃癌风险联系的肠上皮化生评估（OLGIM）分期改善率、萎缩/肠上皮化生组织病理学评分及改善率、血清胃分泌功能水平［胃分泌素-17（G-17）、胃蛋白酶原（PG）Ⅰ、PGⅡ和PGR（PGⅠ/PGⅡ）］、中医证候评分，并进行临床疗效评价。

结果

治疗后治疗组OLGA和OLGIM分期改善率分别为72.94%（62/85）和62.35%（53/85），对照组分别为48.68%（37/76）和38.16%（29/76），治疗组均优于对照组（

＜0.05）。治疗组胃角、胃窦组织病理学评分萎缩改善率优于对照组（

＜0.05），胃体组织病理学评分萎缩改善率及各部位组织病理学评分肠上皮化生改善率两组比较差异无统计学意义。治疗后治疗组PGⅠ水平较治疗前升高，且改善效果优于对照组（

＜0.05）；治疗后治疗组G-17水平较治疗前升高（

＜0.05）。治疗后两组中医证候评分均较治疗前下降，治疗组下降较对照组更显著（

＜0.05）；治疗组中医证候临床总有效率为84.62%（66/78），显著高于对照组的67.24%（39/58，

＜0.05）。

结论

健脾解毒方治疗木村-竹本闭合型CAG脾虚湿热证伴肠上皮化生可以显著降低OLGA和OLGIM分期，改善胃分泌功能水平和中医临床证候。

Abstract

Objective

To evaluate the clinical efficacy of

Jianpi Jiedu Decoction

（健脾解毒方， JJD） in treating Kimura-Takemoto closed-type chronic atrophic gastritis （CAG） with spleen deficiency and damp-heat syndrome accompanied by intestinal metaplasia.

Methods

A total of 180 patients with Kimura-Takemoto closed-type CAG characterized by spleen deficiency and damp-heat syndrome were enrolled and randomly assigned to a treatment group and a control group， with 90 cases in each group. The treatment group received JJD， while the control group was treated with vitamin C and folic acid tablets combined with

Liujunzi Decoction

（六君子汤）. The treatment duration for both groups was 6 months. The improvement rates of the operative link on gastritis assessment （OLGA） staging and the operative link on gastric intestinal metaplasia assessment （OLGIM） staging， histopathological scores and improvement rates of gastric atrophy and intestinal metaplasia， serum gastric secretory function indices such as gastrin-17 （G-17）， pepsinogen （PG）Ⅰ， PGⅡ， and the PGⅠ/PGⅡ ratio （PGR）， traditional Chinese medicine （TCM） syndrome scores， and overall clinical efficacy were evaluated and compared between groups.

Results

The improvement rates of OLGA and OLGIM staging in the treatment group were 72.94% （62/85） and 62.35% （53/85）， respectively， while those of the control group was 48.68% （37/76） and 38.16% （29/76） respectively. The treatment group showed significantly better outcomes than the control group （

＜0.05）. The improvement rate of gastric atrophy based on histopathological scores in the gastric angle and antrum was significantly higher in the treatment group than in the control group （

＜0.05）. No significant difference was observed between groups in the improvement rate of gastric body atrophy or in the improvement r

ates of intestinal metaplasia at various gastric sites. After treatment， PGⅠ levels in the treatment group increased and showed greater improvement than those in the control group （

＜0.05）. The level of G-17 in the treatment group also increased significantly after treatment （

＜0.05）. TCM syndrome scores decreased in both groups， with a more pronounced reduction in the treatment group （

＜0.05）. The overall clinical effective rate based on TCM syndrome evaluation was 84.62% （66/78） in the treatment group， significantly higher than 67.24% （39/58） in the control group （

＜0.05）.

Conclusion

JJD can significantly reduce OLGA and OLGIM staging and improve gastric secretory function indices and TCM clinical syndromes in patients with Kimura-Takemoto closed-type CAG with spleen deficiency and damp-heat syndrome accompanied by intestinal metaplasia.
