摘要

总结凃晋文教授运用“壮火食气”理论从气、火分期辨治癌因性失眠的临证经验。认为癌因性失眠的病机演变过程可划分为三个阶段：初始期气机怫郁、壮火萌动、君火不宁，乃气盛而火不盛阶段，方选三花解郁安神汤以行气达郁、清宣壮火、宁心安神；进展期壮火燔灼、气化衰惫、毒火扰神，乃气火壅盛阶段，方选黄连解毒安神汤以清散壮火、泻火解毒、清心安神；终末期壮火潜藏、食气伤正、心神失养，乃气火皆衰阶段，方选二仁养心安神汤以益气滋阴、交通心肾、养心安神。

Abstract

This paper summarizes professor TU Jinwen's clinical experience in treating cancer-related insomnia （CRI） based on different

and fire. It is believed that the pathogenesis of CRI can be divided into three stages. At the initial stage，

movement is constrained， while the strong fire begins to stir， and the sovereign fire is unsettled， when

is abundant， but the fire is not excessive. For this，

Sanhua Jieyu Anshen Decoction

（三花解郁安神汤） is suggested， which can move

and vent constraint， clear and diffuse strong fire， calm the heart and spirit. At the progressive stage， strong fire becomes intense and burning， and

transformation weakens， with toxin fire harassing the spirit. This is the stage where both

and fire are excessive， for which

Huanglian Jiedu Anshen Decoction

（黄连解毒安神汤） can be used to clear and dissipate strong fire， drain fire and resolve toxin， clear heart and calm spirit. At the terminal stage， strong fire subsides， and consumption of

damages healthy

， with failure of nourishment of heart spirit， when both

and fire deplet

e. Correspondingly，

Erren Yangxin Anshen Decoction

（二仁养心安神汤） is used to boost

and nourish

yin

， restore interaction between the heart and the kidney， nourish the heart and calm spirit.
