摘要

提出了基于辨机论治的“病-机-证-症-法”五位一体诊疗模式，该模式包括识病、辨机、辨证、辨症及形成治法五个方面。识病包括中西医疾病诊断，以把握疾病发展规律；辨机以把握疾病本质特点；辨证以把握疾病阶段特点；辨症以关注患者所急所苦；最终形成包括药物内服、外用及非药物疗法等在内的多元治法。“病-机-证-症-法”五者顺序递进，覆盖诊疗全程，其中识病是诊疗模式之始，辨机是诊疗模式之本，形成治法是诊疗模式之终，以期进一步实现精准化治疗、提高临床疗效。

Abstract

This paper proposes a five-in-one diagnosis and treatment model of "disease-mechanism-syndrome-symptom-method" based on treatment guided by mechanism differentiation. The model consists of five component including disease identification， mechanism differentiation， syndrome differentiation， symptom differentiation， and the formulation of therapeutic strategies. Disease identification refers to the diagnosis of diseases in both traditional Chinese medicine （TCM） and western medicine， aiming to clarify disease progression rules. Mechanism differentiation focuses on grasping the essential characteristics of the disease. Syndrome differentiation emphasizes the stage-specific features of disease evolution. Symptom differentiation highlights the patient's most urgent complaints and suffering. Finally， a comprehensive therapeutic strategy is developed， including internal medication， external applications， and non-pharmacological therapies. The five components of "disease-mechanism-syndrome-symptom-method" are organized in a sequential and progressive manner， forming an integrated whole that spans the entire diagnostic and therapeutic process.Disease identification represents the starting point， while mechanism differentiation serves as the core， and therapeutic formulation is the endpoint of the model， with the aim of achieving more precise treatment and improving clinical efficacy.
