摘要

从“审察卫气，为百病母”入手，认为卫气具有剽悍疾利、动静相依的特点，针刺调气重在调节卫气。故而结合卫气的特点，通过对“一退三进，钻剔四方”操作的分析，得出苍龟探穴针刺法具有促进得气、聚合卫气，布陈卫气、通经解结，动静结合、与气相协的特点。据此阐释苍龟探穴针刺法的立论基础在于调控卫气，并结合临床运用体会总结其操作重点为缓速、守气和低频。缓速指针刺手法操作要和缓，其目的在于聚卫气于针下并促进得气；守气指在得气的基础上通过手法操作增强针刺感应，以提高临床疗效；低频是指低频率（3或5次/min）行针，如震颤、摇法等“钻”“剔”动作，可调动卫气并有助于医者细察针下气感。通过对苍龟探穴针法“一退三进，钻剔四方”的内涵分析，以期为临床中苍龟探穴手法的应用提供参考。

According to the theory of “examining

， and by analyzing the manipulation techniques of “one lifting and three thrusting， accompanied by drilling and scraping in four directions”， it is found that turtle probing the cave needling can promote the arrival of

， unblock the channels and dissipate masses， integrating both movement and stillness， and in conjunction with

. Therefore， the theoretical basis of turtle probing the cave needling is to regulate

wei qi

， and the key points of its manipulation is slow speed， needlingsensation maintaining， and low frequency. Slow speed requires that the needling should be gentle so as to gather

wei qi

and promote arrival of

. Needling sensation maintaining is to enhance the sensation after qi arrival with further manipulation in order to improve clinical effect. Low frequency （3 to 5 times per minute） requires the needling manipulation should be low frequent， such as vibration， handle-shaking and other “drilling” and “scraping” methods， which can mobilize

wei qi

and facilitate the doctors to observe the needling sensation carefully. The connotation analysis of manipulation techniques of “one lifting and three thrusting， accompanied by drilling and scraping in four directions” is expected to provide a reference for clinical application of turtle probing the cave needling.
