摘要

总结杨文明教授分期论治中风后脑水肿的临证经验。认为瘀、毒、水、痰是中风后脑水肿的主要病理因素，根据其不同发病时期、不同脑水肿程度，将其分为三期。早期、轻度脑水肿阶段，多以瘀、毒为主，治宜活血化瘀、解毒通络，方用通窍活血汤合黄连解毒汤加减；极期、中度脑水肿阶段，多以瘀、毒、水为要，治宜化瘀解毒、通络利水，方用自拟化瘀解毒利水方；晚期、重度脑水肿阶段，多以瘀、毒、水、痰并见，治宜豁痰化瘀、通腑解毒，方用自拟加味星蒌承气汤。临证善用虫类药化瘀通络利水；强调整体观念，上病下取，二便分消；同时重视患者意识状态的动态观察和辨识，在病情未加重或恶化之前及时采取措施以截断病势。

Abstract

This paper summarized Professor YANG Wenming's clinical experience in treating cerebral edema after stroke by stages. It is believed that stasis， toxin， water and phlegm are the main pathological factors of cerebral edema after stroke. According to the onset time of the disease and the degree of cerebral edema， it can be divided into three stages. In the early stage （mild cerebral edema）， stasis and toxin are the main pathogenesis， which should be treated by activating blood and dissolving stasis， resolving toxins and unblocking collaterals. Tongqiao

Huoxue Decoction

（通窍活血汤） and

Huanglian Jiedu Decoction

（黄连解毒汤） could be used. At extreme stage （moderate cerebral edema）， stasis， toxin and water are the main factors， and the treatment method is to dissolve stasis and resolve toxin， unblock collaterals and promote urination. Self-made

Huayu Jiedu Lishui Formula

（化瘀解毒利水方） is recommended. In the late stage （severe cerebral edema）， mostly stasis， toxin， water， and phlegm are seen together， which could be treated by eliminate phlegm and remove stasis， unblock the bowels and resolve toxins， with self-made

Jiawei Xinglou Chengqi Decoction

（加味星蒌承气汤）. In clinical practice， he is good at using insects to dissolve stasis， unblock collaterals and promote urination. Following the concept of holism， it is recommended to treat the upper disease from the lower part， and disperse evils of urine and stool separately. At the same time， attention should be paid to the dynamic changes and identification of the patient's state of consciousness， and timely measures should be taken to cut off the progression of the disease before exacerbation or worsening.
