摘要

通过对既往研究及本病发生发展特点分析，提出“阳虚邪中”为中风的根本病机。认为中风病以阳虚邪害空窍为本、风从营卫害表为标、兼气火痰瘀不通为痹，治疗上应以温通阳气为本，以外散表邪为首，以祛除积滞为辅，并根据病程分期治疗。病程初期可治玄府，调和阴阳，方选葛根汤；中期可温通阳气，扶正祛邪，根据阳虚程度由轻到重，分别选用桂枝加附子汤、麻黄附子细辛汤、四逆汤、参附汤；后期可温补阳气，消积通滞，选用侯氏黑散、续命汤类方。

Abstract

By reviewing the previous literature and analyzing the characteristics of the occurrence and development of the disease， it is proposed that “

yang

deficiency and pathogen attack” is the fundamental pathogenesis of stroke （wind-strike）. The root cause is

yang

deficiency and pathogen invading the orifice， while the branch cause is wind invading the exterior from

ying

and

wei

（营卫） levels with obstruction of

， fire， phlegm and stasis leading to

（痹） condition. Therefore， it is emphasized that the fundamental treatment should be warming and unblocking

yang

； dispersing exterior pathogen should be taken as the first priority， while eliminating stagnation as a supplement， and staged treatment according to the disease course is suggested. At the early stage of the disease， it is advised to treat sweat pores and harmonize

yin

and

yang

with

Gegen Decoction

（葛根汤）. At the middle stage， the method of warming and unblocking

yang

， reinforcing healthy

and dispelling pathogen could be used， and

Guizhi Fuzi Decoction

（桂枝加附子汤），

Mahuang Fuzi Xixin Decoction

（麻黄附子细辛汤），

Sini Decoction

（四逆汤） or

Shenfu Decoction

（参附汤） is selected according to the severity of

yang

deficiency from mild to severe. At the late stage， it is suggested to warm and supplement

yang qi

， disperse accumulation and eliminate stagnation with

Hou's Hei Powder

（侯氏黑散） and Categorized Formula of

Xuming Decoction

（续命汤）.
