摘要

目的

总结针刺临床机制研究国内外发文现状及研究热点、趋势。

方法

检索中国知网（CNKI）和Web of Science（WOS）数据库收录的针刺临床机制研究相关文献，检索时间范围为2001年1月1日至2022年10月30日，提取纳入文献的题名、作者、单位、文献来源、关键词、摘要、发表时间等信息建立数据库，应用CiteSpace 5.6.R2软件进行作者、机构共现分析，关键词聚类和突现分析，并绘制可视化图谱；同时对纳入研究的试验设计进行描述性分析。

结果

共纳入337篇文献，其中英文文献106篇，中文文献231篇。针刺临床机制研究领域核心作者共56人，发文量203篇。中国中医科学院与国内外多所研究机构形成了紧密的合作网络，但尚未形成核心作者和机构。337篇文献中有253篇样本量在30～100例，264篇文献涉及随机方法，发文量排名前五位的疾病类型分别为脑卒中（46篇）、抑郁症（31篇）、肥胖（16篇）、失眠（14篇）和功能性消化不良（14篇），设置盲法的文献有70篇，有326篇文献评估了基线期均衡性，35篇文献提及随访，单中心随机对照试验291篇。对关键词进行聚类分析结果显示，在图谱中形成了11个中文文献聚类标签和13个英文文献聚类标签，表明热点疾病类型包括炎症、疼痛类疾病，心理/精神类疾病，肿瘤、脓毒症等急危重症等；热点机制类别主要为针刺抗炎、镇痛与应激反应。对关键词突现进行分析共得到10个中文突现词和8个英文突现词，关键词聚类时区/时线图及突现结果表明前沿趋势涉及心理/精神类疾病的防治研究、抗炎镇痛的分子靶标、精准个体化诊疗机理和临床试验的质量控制。

结论

针刺临床机制研究领域尚未形成核心作者和机构，试验设计的质量有待提高。研究内容涵盖多种疾病类型和机制类别，其中心理/精神类疾病、抗炎镇痛、精准个体化诊疗的机理探索等是当前的研究热点和前沿趋势。

Abstract

Objective

To summarize the current situation and hot trends of the relevant literature of acupuncture clinical mechanism researches.

Methods

CNKI and Web of Science （WOS） were searched from January 1st， 2001 to October 30th， 2022 for literature on clinical mechanism of acupuncture. The title， author， institution， literature source， keywords， abstract， publication time and others were extracted to establish a database. Using CiteSpace 5.6 R2 software， the co-occurrence analysis of authors and institutions， keyword clustering and emergence analysis were performed， and a visual map was drawn. At the same time， descriptive analysis was carried out for the experimental design included in the study.

Results

A total of 337 papers were included， involving 106 in English and 231 in Chinese. There were 56 core authors in the field of acupuncture clinical mechanism research， who have published 203 papers. China Academy of Chinese Medical Sciences has formed a close cooperation network with many research institutions at home and abroad， but it has not yet formed a core author and institution. Of the 337 papers， 253 papers had a sample size of 30-100 cases， and 264 papers used randomization. The top five diseases explored were stroke （46 papers）， depression （31 papers）， obesity （16 papers）， insomnia （14 articles） and functional dyspepsia （14 articles）. Seventy papers set blinding， and 326 papers evaluated the baseline balance； 35 papers mentioned follow-up， and there were 291 single-center randomized controlled trials. Cluster analysis of keywords obtained 11 clustering labels from Chinese literature and 13 clustering labels from English literature， indicated that the hot issues on diseases included inflammation and pain， psychological/ psychiatric diseases， tumors， sepsis and other acute and critical diseases， and the focused mechanism were acupuncture anti-inflammatory， analgesia and stress response. The analysis of the emergence of keywords yielded a total of 10 Chinese and 8 English emergence words. Keyword clustering time zone/ time line diagram and emergent results showed that cutting edge trends involved prevention and treatment research on psychological/ psychiatric diseases， molecular targets of anti-inflammatory and analgesic effects， precise and individualized diagnosis and treatment mechanism， and quality control of clinical trials.

Conclusion

Core authors and institutions have not yet formed in the field of acupuncture clinical mechanism research， and the quality of related research design needs to be improved. The content covers a variety of diseases and mechanism categories， and the current hot issues and frontier trends are psychological/psychiatric diseases， anti-inflammation and analgesia， and exploration of precise individualized diagnosis and treatment mechanism.
