摘要

肿瘤相关性贫血（CRA）发病率高且影响预后，临床有效治疗率较低且现代医学治疗手段有限。五脏生血是根于经典的中医理论，揭示了五脏在血液生成过程中的重要作用。立足于“五观辨治”体系，提出以“未病观”聚焦易感人群、并通过饮食调护，先安未受邪之地；以“时空观”分病期、分阶段、分病种、分部位辨治CRA；以“症状观”应用角药、对药缓解主要症状；以“精准观”探索现代医学指标与中医证型的内在关联，富集靶标定向防控CRA。并且基于五脏生血的“核心观”，分层论治不同阶段的CRA：运用健脾益肺、补气生血法治疗术后CRA，运用养肝益肾、养血填精法辅助CRA化疗，运用清热化瘀、养阴生髓法辅助CRA放疗，运用养心安神、抗癌解毒法治疗晚期CRA。

Abstract

Tumor-related anemia （CRA） has a high incidence rate and significantly affects prognosis. However， the clinical effective treatment rate is low， and modern medical treatment options are limited. "The five

zang

（脏） organs gene-rating blood" theory， rooted in classical traditional Chinese medicine （TCM）， highlights the crucial role of the five

zang

organs in blood production. According to the system of "diagnosis and treatment based on five viewpoints"， this article has proposed the following five approaches. The "prevention viewpoint" focuses on high-risk populations and recommends dietary adjustments to safeguard the area not yet affected by pathogens； the "time and space viewpoint" emphasizes treatment of CRA based on the differentiation of disease stage， type， and location； the "symptom viewpoint" suggests corresponding three-herb or paired herb combination to relieve major symptoms； and the "precision viewpoint" explores the internal relationship between modern medical indicators and TCM syndromes to target and prevent CRA more effectively. Additionally， based on the core theory of "five

zang

organs generating blood"， staged approach is used to treat CRA. For postoperative CRA， the method of fortifying the spleen and benefiting the lung， invigorating

and blood should be used. To assist with CRA related to chemotherapy， it is suggested to tonify the liver and kidney， nourish blood and supplement essence. For CRA related to radiotherapy， the method of clearing heat and dissolving stasis， nourishing yin and generating marrow can be used. To treat advanced-stage CRA， it is better to nourish the heart and calm the mind， fight cancer and resolve toxins.
