摘要

糖络病学说认为糖尿病血管病变的关键病机为久病入络，且存在“络滞-络瘀-络闭络损”的动态病理进程，对于泛血管疾病的防治具有指导作用。以糖络病学说系统阐释脉络与泛血管在网络层级结构、物质能量输布功能及“功能异常-器质损伤”病理层次等方面的联系，并指出谷道（消化系统）、气道（呼吸系统）、水道（泌尿系统）与血道（循环系统）“四道”功能异常是泛血管疾病发生发展的重要因素。临证强调泛血管疾病未病时从“四道”入手干预，早期治络以延缓血管衰老；既病时以“全程通络、脉络分治”为核心治法，针对大血管病变（脉病）以化斑汤破瘀化痰散结，针对微血管病变（络病）以仝氏微血管共病方益气养阴通络，并结合宏观调态与微观打靶的思路灵活用药。

Abstract

Sugar collateral disease theory reveals that the key pathological mechanism of diabetic vascular lesions lies in the concept of "prolonged illness affecting collateral"， involving a dynamic pathological progression of "collateral constraint-collateral stasis-collateral blockage-collateral damage"， which provides insights and guidance for the prevention and treatment of pan-vascular diseases （PD）. This paper systematically elucidates the relationships between the vascular collaterals and the pan-vascular system in terms of their network hierarchy， functions of material and energy distribution， and the pathological progression from functional disorder to organic damage based on the sugar collateral disease theory. It further points out that dysfunctions of the four pathways， that is grain tract （digestive system），

tract （respiratory system）， water tract （urinary system）， and blood tract （circulatory system）， are crucial factors contributing to the onset and development of PD. In clinical practice， it emphasizes

that intervention focusing on "four pathways" for PD should be performed before disease onset， effectively delaying vascular aging via early treatment of the collaterals. Once PD develops， it follows the principle of "unblocking collaterals throughout the entire course and implementing differential treatment for vessels and collaterals". For macrovascular lesions （vessel diseases），

Huaban Decoction

（化斑汤） is used to dispel stasis， resolve phlegm， and dissipate nodules； for microvascular lesions （colla-teral diseases），

Tong's Microvascular Comorbidity Formula

（仝氏微血管共病方） is employed to invigorate

， and unblock collaterals. These regimens are further modified by integrating macroscopic state regulation with local targeted therapy.
